Digitale Totalprothesen – Möglichkeiten und Grenzen

Das digitale Zeitalter hat nun auch die Herstellung von Totalprothesen erreicht. Bei der CAD/CAM-Fertigung von Totalprothesen gibt es 2 Wege: das innovative Behandlungskonzept, also Abformung und Kieferrelationsbestimmung in einer Sitzung, oder der gewohnte klinische Ablauf über Wachsprobe und anschließende Fräsung der Prothesenbasen. Der Beitrag geht insbesondere auf die neuen Herstellungswege ein und nimmt einen Vergleich zur konventionellen Prothesenanfertigung vor.

Kernaussagen
  • Es gibt derzeit mehrere Anbieter von digitalen Prothesen. Die zur Herstellung von CAD/CAM-Prothesen notwendigen klinischen Schritte sind bei den Anbietern unterschiedlich. Bei manchen Systemen können die Prothesen bereits in der 2. Sitzung eingegliedert werden, was aber eine Änderung der gewohnten klinischen Abläufe bedeutet.

  • Auffällig ist bei digitalen Prothesen insbesondere der bessere Prothesenhalt, der sich durch Wegfall der Polymerisationsschrumpfung erklären lässt.

  • Weitere Vorteile von CAD/CAM-Prothesen sind neben der verkürzten Behandlungszeit auch die besseren Materialeigenschaften sowie die problemlose Herstellung von Duplikatprothesen.

  • Die vollständige digitale Herstellungskette ist zwar noch nicht möglich, doch sind erste Teilschritte in den noch fehlenden Bereichen in Entwicklung. So ist die digitale Abformung derzeit nur im Oberkiefer ausreichend genau machbar.

  • Die zentrische Kieferrelationsbestimmung ist jedoch digital nicht durchführbar und es ist fraglich, ob diese jemals digital möglich sein wird.
